On Thursday, June 24, 2010, a 30 ft. long concrete panel above the exit to a parking garage at Milwaukee's O'Donnell Park fell as a group of people were walking below, killing a 15 year-old boy and injuring two others.
Weeks after the incident, it was revealed that each of the decorative panels were attached to the structure by only two connectors, instead of the four connectors called for in the building's original design.
The area was much busier than normal due to it being opening day of the nearby annual Summerfest music festival. The 11-day festival averages over 75,000 visitors each day.
In November 2013, a jury found the company which installed the panels responsible for the accident. Advance Cast Stone was ordered to pay nearly $39 million to the victims and the county.
