Before attending to Napoleon, Arnott served in Holland, Minorca, Malta, Sicily, Calabria, Austria, Spain with the Duke of Wellington, and India. In 1821 he arrived in St. Helena and quickly established a rapport with the Emperor. Napoleon was so fond of the doctor that he ordered a snuff box with "N" carved on it. Napoleon also gave him a locket of his hair, a pair of dueling pistols and a handkerchief. Arnott was with Napoleon until the latter's death on May 5th, 1821, taking a death mask and the autopsy.
