On April 27, 1944, Lockheed Ventura 2218 of the No. 115 sqr RCAF took off from RCAF Patricia Bay. Its destination was RCAF Tofino. It never made it there. I couldn't find much about this crash, other than that it was night at the time. Due to the lack of evidence available, I'd say it was likely a pilot error.
