The city of Worcester lists the Pine Hill Reservoir (one of the main sources of Worcester's water) as being in three towns (Paxton, Rutland & Holden.)
As for the dam itself - the USGS doesn't list that dam (just one if their random omissions, i guess) but I did find one (official enough?) piece of evidence giving it the name Pine Hill Reservoir Dam; a file listing that name is online within the U.S. Environmental Protection Agency's Watershed Information Network... no other name for that dam could I find.
Actually just found another piece of evidence on an inventory list of High Hazard Dams for Massachusetts which lists the dam in the town on HOLDEN, Mass.
