Santa Barbara Airlines - #518 Crash

The ATR 42-300 crashed shortly after takeoff from Merida, Venezuela on a flight to Caracas. It crashed into a mountain on 21 February 2008; killing all on board: 43 passengers and 3 crew. The accident is the highest death toll involving an ATR 42.

The investigation revealed that the crew did not properly initialize the flight naviagation system. More info at link below.
